How do I get my 5 year old son to start going poop on the toilet?

My son has been 50% potty trained for at least 2 months now. He is going to be 5 years old this October. However, for some reason, I absolutely CANNOT get him to go Number 2 on the toilet. He pees standing up with no problem at all and goes all by himself, however, he has been pooping his underwear all the time. I have tried asking his doctors office for advice and they have told me to sit him on the toilet with a book for a half hour few times during the day OR a half hour after he eats. I do this, he does not go. Then after he is off the toilet if he had to poop, he goes in his underwear 10 minutes later. I have taken toys away and that doesn't work. I have tried EVERYTHING I can think of. I don't know if he is afraid to sit on the toilet since he pees standing up? And when he does poop himself, I ask him why he didn't come to me and tell me he had to poop and he tells me that he wanted someone to sit with him in the bathroom. I have no problem doing that but he still doesn't tell me when he has to go. Does anyone have any advice for me. I don't know what to do at this point. He starts Kindergarten next year and I am afraid he wont be 100% potty trained by then. Thank you.
